Gynecomastia

Gynecomastia refers to an enlargement of the breast gland tissue in men, often bilateral, firm, and tender. It may be idiopathic, related to a hormonal imbalance, certain medications, or associated with weight gain and pseudogynecomastia (adipomastia). This aesthetic and sometimes painful condition is frequently difficult to cope with, particularly for adolescents.
When no cause is identified or the discomfort persists despite possible treatments, surgery may be proposed to restore a harmonious chest and a more masculine silhouette.

Gynecomastia
Before the Procedure
A preoperative assessment is performed according to the surgeon's prescriptions.
A consultation with the anesthesiologist takes place no later than 48 hours before the procedure.
Certain recommendations must be followed:
- No anticoagulants or products affecting blood clotting should be used, unless otherwise advised by the anesthesiologist.
- Smoking cessation is required 3 to 4 weeks before the procedure, due to its impact on healing.
- The anesthesiologist will provide specific instructions for this procedure.
The Surgical Procedure
The procedure is performed under general anesthesia.
- An incision at the lower edge of the areola allows removal of the breast gland tissue and excess fat.
- In cases of fatty gynecomastia, liposuction alone may suffice, with very small scars.
- When there is significant skin excess, longer scars may be necessary (peri-areolar, horizontal, or even areola grafting in extreme cases).
- A drain may be placed depending on the surgeon's practice.
- A shaping dressing and a compression vest (bolero-type) are applied at the end of the procedure.
The duration of the surgery ranges from 1 to 2 hours.
After the Procedure
Pain is managed with analgesics. Swelling, bruising, and difficulty raising the arms are common.
- The initial dressing is removed after a few days and replaced with a light dressing and a compression vest to be worn day and night for 3 to 4 weeks.
- Time off work varies from 5 to 21 days depending on the case.
- Resuming sports is recommended after 2 months.
Scar evolution:
- Scars evolve over 12 to 18 months: initially fine, then pink, before gradually fading.
- They must be protected from the sun during the first year.
